For your first appointment we would gladly like to ask you to provide us with a list of document as seen below, with information about your health:

• The letter of your referring doctor or dentist.
• A list of your current medications.
• Medical imagery, even old ones (X-ray, CT scan, ultrasound, MRI).
• Dental casts (if available), even old ones.
• Insurance documents and/or documents in equivalence of your insurance information, if available.
